Forms and general information
- Intake form Word document: allows you to fill out your intake before your visit.
- Pediatric Intake Word document: intake for 16 y.o. and under.
- Release of Records Word document: take or mail this to previous practitioners.
- Diet diary Excel document: often it is helpful for us to see what your typical diet is like.
- Fees and hours Word document: Our hours are Mondays 11-3, Tuesdays - Fridays 9-5. Our fees are competetive with all other Naturopathic Doctors in the area, including Denver (Physical exam $45; Pediatric under 16 return $60, initial $120; Adult return $80, initial $220 - if it's a simple case, we charge the return rate for an initial visit).
